REGISTRATION, GENERAL INFO, MEDICAL FORMS
In order to be enrolled, students
must have completed:
- A Registration Form and paid the registration
fee. The registration fee is refundable
only if you move from the area before the school year begins.
- A GENERAL INFORMATION form completely filled out (names, complete addresses and phone numbers) and with a current photograph
attached. Please notify the office of
any changes: i.e. phone number, cell
number or work number. All students, new and returning, need to compete and return this
form to school on or before the first day of school.
No child will be admitted without this form.
- The VIRGINIA STATE MEDICAL form must be returned by the end of
September or 30 days after enrollment. If your child was enrolled
with us last year, it is not necessary to fill out a new Virginia State Medical form
unless your child will be
attending as a kindergartener. Then a new
form is required.
